Digitizing Patient Safety Excellence Across Egypt's Hospitals
A national digital platform delivering the complete PSFHI 3rd Edition framework — 134 criteria across 5 domains — with real-time scoring, automated reporting, and structured action planning.
From Paper Checklists to a Living Digital Platform
The Patient Safety Friendly Hospital Initiative (PSFHI) is led in Egypt by WHO Egypt in partnership with the Ministry of Health and Population. Based on the WHO framework first launched in 2011 and now in its 3rd Edition, the initiative encompasses 134 criteria across 5 domains, assessed through document review, structured interviews, and facility observation.
This digital platform replaces the current paper-and-Excel workflow, enabling WHO-trained assessors to conduct structured hospital assessments on tablets or laptops — online or fully offline — with automated scoring, real-time aggregation, and one-click report generation.
The PSFHI is not an accreditation system. It is a supportive recognition programme focused on building capacity, identifying gaps, and facilitating continuous improvement in partnership with hospital teams.
Assessment Phases
- 1Pre-Assessment — Hospital self-assessment & document preparation (6 months before)
- 2On-Site Survey — 2–4 day visit by WHO-trained assessor team (documents, interviews, observation)
- 3Scoring & Review — Real-time criterion scoring (0 / 0.5 / 1 / NA) with gap comments and corrective actions
- 4Exit Report — Auto-generated summary of findings, recommendations, and certification level
- 5Action Planning — Hospital responds to findings with corrective actions, due dates, and responsible owners
- 6Recognition — Certification Level 1–4 awarded; reassessment every 2–3 years
5 Domains · 21 Standards · 134 Criteria
The PSFHI 3rd Edition framework covers every dimension of patient safety from governance and leadership to clinical practice, environment, and continuous learning.

Everything Assessors, Hospitals, and Admins Need
Built to replace the paper-and-Excel workflow end-to-end.
Offline-First Scoring
Complete a full assessment with zero internet connectivity. Data syncs automatically when connectivity is restored — essential for field assessments.
Real-Time Score Aggregation
Every criterion scored instantly updates Standard, Domain, and Hospital totals — including Critical, Core, and Developmental breakdowns — with no manual calculation.
Automated Reports
Generate the full assessment report — cover, score hierarchy, charts, observations, recommendations, action annex — in PDF, Word, HTML, or Excel with one click.
Hospital Action Plans
Hospitals receive their results, review assessor recommendations, set corrective actions with due dates and responsible owners, and track remediation progress.
Portfolio Dashboard
Program administrators compare hospitals across all metrics, track national trends, monitor action plan compliance, and generate portfolio-level reports for MoHP and WHO.
Role-Based Access Control
Assessors, hospital users, and organization admins each see exactly what they need. Scores are locked on admin approval; hospitals get view-only access on submission.
Multi-Language Support
Arabic (RTL) for hospital-facing screens and reports. English and French for assessor and admin screens — enabling regional scale-up beyond Egypt.
Evidence Capture
Attach photos or documents to any criterion during the assessment. Evidence is stored securely and appears in the final report and report annex.
Self-Assessment Module
Hospitals conduct structured self-assessments between official cycles using the full PSFHI framework, receiving a readiness report to prepare for the next survey.
Designed for Every Stakeholder
WHO-trained technical officers conducting 2–4 day on-site hospital assessments as part of a multidisciplinary team. Need to navigate all 134 criteria, score in real time (online or offline), attach evidence, and generate an exit report before leaving the facility.
- ✓Navigate framework by Domain → Standard → Criterion with progress tracking
- ✓Score each criterion: Fully Met (1) / Partially Met (0.5) / Not Met (0) / NA
- ✓Optional: score Measurable Elements for derived criterion scores
- ✓Mandatory gap comments for any score below Fully Met
- ✓Attach photos or documents as evidence per criterion
- ✓Live score panel: Overall, Critical, Core, Developmental, per Domain
- ✓Aggregated Review Screen: assign corrective actions and priorities for all gaps
- ✓Auto-generate Exit Report; retain edit access until admin approval
- ✓Full offline mode on web (PWA) and native mobile apps
Four Levels of Patient Safety Achievement
Based on scoring across Critical, Core, and Developmental criteria. All 25 Critical criteria must be Fully Met before any certification level can be awarded. (Source: WHO PSFHI Manual, 3rd Edition — Table 5)
- ✓All 25 Critical criteria: 100% (Fully Met)
- ✓Core criteria: Any percentage
- ✓Developmental criteria: Any percentage
- ✓All 25 Critical criteria: 100%
- ✓Core criteria: 60–89%
- ✓Developmental criteria: Any
- ✓All 25 Critical criteria: 100%
- ✓Core criteria: ≥90%
- ✓Developmental criteria: Any
- ✓All 25 Critical criteria: 100%
- ✓Core criteria: ≥90%
- ✓Developmental criteria: ≥80%
Reassessment: Level 1–2 every 2 years · Level 3–4 every 3 years
